Case Management links patients with health care, psychosocial support, and other services. The coordination and follow-up of medical treatments is an important role in medical case management. It also includes treatment-adherence counseling to ensure readiness for, and adherence to, complex HIV/AIDS medications.
Case management services at SCC are a component of the Multi-Disciplinary Team (MDT) approach. The MDT consists of the patient, medical provider, nurse, counselor, and case manager. The MDT may also include a psychiatrist, peer advocate, home health nurse, or other important people specific to the patient's needs. Throughout the process, SCC staff maintains a commitment to patient confidentiality.
Behavioral health counseling by licensed mental-health professionals is also available. Areas of counseling may include, but are not limited to, dealing with a recent HIV diagnosis; adapting to HIV/AIDS; mental and emotional distress; grief and loss; trauma; career development and workforce re-entry; substance use/abuse; relationship issues; and gender identity issues.
No one will ever be denied treatment at SCC because of inability to pay. What we require is: proof of your HIV status, an address in New Mexico, and proof of income. There are many ways to receive help in paying for your care including:
New Mexico Department of Health HIV/AIDS Care and Services Program
The New Mexico Department of Health, Infectious Disease Bureau, has contracted with regional HIV specialty-care agencies throughout the state. The organizations are funded to provide medical care, case management, home health care, mental-health and substance-abuse services, housing assistance, food bank assistance, travel reimbursement, and other support services. SCC is one of the providers. Case managers assist our clients with the eligibility and enrollment in the program. Travel reimbursement is available for New Mexico residents who are living with HIV for travel to and from HIV-related medical, dental, social service, mental health, substance-abuse treatment and support group appointments.
The AID & Comfort Fund
This fund is supported by private donations and is used to pay for medical bills and contribute to expenses and other needed support services that are not covered by insurance or other programs. One key of this fund is to allow for discounts on medication co-pays when using our pharmacy. Proceeds from the AID & Comfort Gala, SCC's major annual fundraiser go directly to the AID & Comfort Fund.

The New Mexico Department of Health provides funding to assist individuals with insurance premium payments and assistance with HIV-related medication co-pays through the Insurance Assistance Program (IAP).
Housing Opportunities for People With AIDS (HOPWA)
The main focus of the housing program is to support people in moving toward independent living and financial stability or as a support for short-term, emergency assistance with housing needs during a health crisis or other unexpected situation. HOPWA is funded by the United States Department of Housing & Urban Development (HUD).
