Case Management & Prevention : Case Management

All clients receive initial case management services upon enrollment with Southwest CARE Center. On the first visit, the client meets with a case manager to complete enrollment paperwork and identify needs related to health insurance, access to medications, and other services. This first meeting with the case manager provides the client with an opportunity to learn about services, determine eligibility for programs, ask questions, and for the client and case manager to begin to get to know each other.
