Medical Case Management & Prevention

The medical case management department at Southwest CARE Center strives to provide supportive services to all our clients as part of a Multi-Disciplinary Team. Each new client is offered the opportunity to meet with and be assigned to a case manager.

The case manager can work closely with the client to improve quality of life by identifying needs and developing plans to meet the daily challenges of living with HIV.
