Case Management

When you’re doing well, everybody benefits. Our case managers assist with the planning, facilitation, and coordination of resources and services, promoting your and your family’s safety, quality of life, and overall wellbeing. We help identify appropriate benefits, financial resources, providers and facilities, will make referrals, and arrange for any services needed.

We begin with an assessment of the patient’s needs, including a realistic evaluation of their environment, and an understanding of their physical, cognitive, financial, and social situation. A comprehensive plan is developed that includes all needed services within the home, community, and care facilities. Professional counseling is also offered to the patient and their family during times of transition.
