The Community Services Department (CSD) represents the cornerstone of our multidisciplinary approach to community health and includes community outreach programs.
We recognize that environment impacts a person’s capacity for wellness and that health conditions like HIV and HCV exist in a living context. The CSD understands the social determinants of health and we engage clients in a holistic manner to address bio psychosocial needs and strengths. With a staff of 13, we provide comprehensive case management services for approximately 600 people living with HIV/AIDS. CSD staff also conducts health outreach and education and rapid HIV and HCV testing in community-based sites in northern New Mexico.
Case Managers can assist you with your health care needs, money management and benefits, insurance issues, housing and nutrition assistance as well as linkage to care.
We offer a variety programs and services that help decrease the transmission of HIV and Hepatitis C.
(It's Better to Know) - An award winning bilingual HIV and Hepatitis C prevention campaign to increase testing, reach those who are positive before their illness becomes life threatening, engage them in treatment and educate them on measures to prevent transmission.
Infected persons are identified, their partners are notified of a possible exposure in a confidential manner, and both the infected person and their partners are provided with treatment, prevention information, and outside referrals as needed.
