The mission of the diabetes team at Southwest Care Center is to provide quality, comprehensive, diabetes self-management education. We believe that education empowers people with diabetes to better manage the disease, avoid potential complications, and achieve optimum health.

Adult and Pediatric Diabetes Prevention and Management Education

Our Diabetes Program begins with an interview to assess your needs. Afterwards, we review your labs and findings, and together we develop a detailed diabetes self-management plan.

Our treatment program uses evidence-based guidelines. We measure your personal, clinical outcomes as well as the clinical outcomes of the program. Our certified insulin pump trainer is also available to help you integrate your insulin pump therapy into your diabetes management plan.

  • Insulin pump training

The prestigious American Diabetes Association Education Recognition Certificate for a quality diabetes self-management education program was recently awarded to the Southwest CARE Center’s Diabetes Self-Management Education Program in September 1, 2015.  ADA believes that this program offers high-quality education that is an essential component of effective diabetes treatment.  To read more, check out our recent blog.
