Behavioral Health Services at Southwest Care

At Southwest Care, we believe in treating the whole person, which is why we follow an integrated-care model – providing primary care, psychiatric care and social services all under one roof. As a result, each of our clinics has a counselor, case manager, and access to psychiatry in addition to the usual medical personnel (doctors, nurses, medical assistants, etc.) available on-site.

Having all these professionals together in one place, ready to work together to bring you the highest quality healthcare possible is unique in today’s healthcare environment. Our behavioral health professionals work closely with our primary care team, providing you with comprehensive care that includes medication management, mental health counseling and case management support.

We are proud to offer these services to all patients at Southwest Care Center and believe that addressing behavioral health concerns is as important as anything else we do.

What is Case Management?

We know that true health and wellness cannot be achieved if you are struggling with some of life’s toughest challenges. We have case managers at each of our clinics to support you all along the way. Case managers act as a trusted guide for patients, helping them access the services they need both within their community’s healthcare system and through local resources.

How Can Case Management Help Me?

Case managers are available at every Southwest Care Center clinic, and specialized services are available to those living with HIV or hepatitis C. Our integrated model of care is designed to holistically meet many of your healthcare and personal needs. An example of what case managers can assist with include:

  • Coordination and follow up of healthcare
  • Development of life skills and/or employment readiness
  • Accessing public benefits such as food stamps or Medicaid
  • Navigating health insurance options
  • Providing community resources and referrals
  • Offering individual or group support
  • Addressing housing goals
  • Enrollment in the Ryan White program for HIV-positive individuals who meet eligibility criteria
